Classification Minimum Requirements:

Bachelor's degree in an appropriate area and four years of relevant experience.

Job Description:
  • Assigns newly enrolled clients to case managers in a manner that will balance the workload of the care services staff members while also ensuring that all clients receive the level of support and attention they need.
  • Monitors client-related record keeping and case notes for accuracy, completeness and compliance with relevant patient/client privacy standards.
  • Establishes and maintains productive collaboration and strategic affiliations with providers of health and social services in Jacksonville TGA to ensure that clients have effective access to health care, housing, mental health services, substance abuse treatment and other relevant services.
  • Takes steps to ensure compliance with requirements imposed by funding sources including accurate record keeping and timely reporting.
  • Participates in activities to educate the community, especially local health and social services organizations, and including health fairs and other events on HIV/AIDS and about the role and services of UF CARES.
  • Oversees and/or directs employee management activities within UF CARES Medical Case Management program staff including interviewing, hiring, training, counseling, and disciplinary actions. Routinely conducts staff meetings to discuss operations, staff issues and customer satisfaction; reviews new policies and
    procedures and shares general information.
  • Participates in various organizational management meetings and committees: Ryan White Committees, Clinical Effectiveness Group (CQI), HIV classes, Leadership Development Institute, Director’s meetings and ensuring the services for PAC Waiver.
Expected Salary:

$59,000 to $70,000 annually depending on education and experience.

Required Qualifications:

Bachelor's degree in an appropriate area and four years of relevant experience.

Preferred:
  • Master's degree in a relevant discipline such as social work, nursing, public health or public administration, and four years of appropriate experience.
  • Bachelor's degree in an appropriate area of specialization and six years of appropriate experience.
  • HIV/AIDS and substance abuse prevention or service coordination experience with the target population preferred.
