Job Description:

Community-Based Education and Outreach
- - Work together with school nurses and health liaisons at current partner schools to ensure successful return of dental treatment consent forms.
- Develop engaging strategies with school personnel to inform students and parents about school-based dental outreach activities.
- Assist with preparing dental outreach paperwork in advance of scheduled activities.
- Provide tailored oral hygiene and nutrition education to children and families.
- Schedule patient appointments and send appointment reminders for community-based dental outreach activities.
- Follow up with families to schedule recall appointments for their child.
- Answer patient phone calls as received during community clinical events.
- Co-develop and disseminate community-facing materials related to oral health.
- Attend health and community fairs to promote the UFCD community dental outreach program and provide oral health education.
- Inform community partners about upcoming UFCD Community Dental Outreach events.

Community Based Clinical Navigation
- Make community-based referrals to agencies and clinics within the dental safety net community of Alachua County.
- Establish and maintain close relationships with safety net organizations and have good working knowledge of key contacts, eligibility criteria, referral process, and follow-up outcomes of these organizations.
- Follow up with patients after referrals have been made to ensure patients are able to access specialty appointments.
- Assist families in enrolling their child in Medicaid and completing forms, as needed.
- Educate families on their child’s dental benefits, if applicable.
- Organizes transportation for patients transferred to or seeking services at UFCD Dental Outreach Programs or Clinics.

Administration
- Attends meetings as appropriate and meet regularly with supervisor to exchange pertinent information and receive supervision.
- Prepares and maintains client and program records in accordance with applicable standards and regulations, grant requirements, etc.
- Verifies patient Medicaid eligibility and coordinates patient billing activities.
- Participates in continuing education activities, remaining knowledgeable in area(s) of expertise.
- Completion of training and ongoing certification as required.

Expected Salary:

$41,760, commensurate with education/experience

Minimum Requirements:

Bachelor's degree in in an appropriate; or an equivalent combination of education and experience.

Preferred Qualifications:

- CPR certification
- Ability to drive to various locations within Alachua County
- Good communication skills to foster relationships with partnerships of outreach program
- Experience working with pediatric populations
- Exhibits cultural sensitivity with diverse racial, ethnic, and socioeconomic populations
- Past experience working in the Medicaid system
- Past experience working in a dental outreach program
- Familiarity with community resources, social services, and healthcare in Alachua County
- Ability to properly demonstrate and communicate health education activities
- Fluent in Spanish
