Job Description:
  • Collaborates with a team and staff to bring the patient and family voice to the forefront to enhance systems level improvement. Share lived experiences to educate teams, staff, and emerging professionals on the advocate-family perspective related to the intended initiative (e.g., quality improvement, strategic
    planning, technical assistance, and training programs, etc. Provides recommendations on how to incorporate the patient and family voice – why should we engage, what does engagement look like, how do we do it?
  • Supports patients in navigating the clinic system and community resources.
  • Promotes and ensures a patient-and family-centered system of care is modeled in health services. Participates in Community Resource Group, CEG meetings, and other applicable meetings.
  • Provides individual and group educational skill building opportunities to foster adherence to medications. Connects with patients and families to provide support, resource referrals, and technical assistance. Participates in community education about family-professional partnerships, or information on patient and
    family-centered care, or the specific initiative with which they are involved.
  • Enhances engagement in care and adherence by conducting patient reminder calls, notification, and follow-up per program policy and procedures.
  • Provides outreach and intensive care coordinator services for HIV/AIDS adolescents. Engages with self-advocates and families to help increase and incorporate more voices of those with lived experiences into systems development and improvement. Engages with other family partners and leaders to identify best
    practices, evaluation tools, and quality improvement activities related to the initiative or work.
  • Adheres to all confidentiality policies.
Expected Salary:

$16 to $18 per hour depending on education and experience.

Required Qualifications:

High school diploma and one year of experience working with children or families.

Preferred:
  • At least 1-2 years' experience with HIV population either via work environment or participation in volunteer activities.
  • Basic computer proficiency.
