The University of Florida (UF) Department of Epidemiology seeks compassionate, detail-oriented individuals to function as Community Health Workers (CHWs) in Putnam and Union Counties. This is a part-time in either Putnam County or Union County and requires the CHW to meet residents in various community locations such as libraries, parks and laundromats to conduct health assessments, provide health education and link residents to community resources and research studies based on their completed health assessment. All information will be kept confidential. CHWs will take initiative but work under supervision according to a prescribed protocol. CHWs must have a valid driver’s license and drive their own vehicle to sites for outreach. The CHWs will also use their own vehicle to provide transportation to for eligible HealthStreet participants to and from non-emergency medical appointments, pharmacies, and other health-related sites. This job requires a genuine regard for diversity, an outgoing personality and attention to detail. This is not an office job. It requires being out in the community in a fast-paced environment requiring detailed record-keeping while offering person-centered support and assistance.
Implements the UF HealthStreet protocol with fidelity, including:
- Engages residents on a daily basis, which may include evenings and weekends, to obtain informed consent, administer the health assessment/intake questionnaire, provide health information and referrals to social and medical resources and opportunities to engage in research..
- Maintains proper documentation and recordkeeping, including daily progress reports and weekly summaries of locations, enrollments and transportations.
- Transports eligible participants to health-related visits as needed.
- Attends weekly HealthStreet meetings
$15.00 per hour
Applicants must have at least a high school diploma or equivalent and a valid Florida driver's license at the time of hire.
Candidates should live in and be familiar with either Putnam or Union County; residency in one of these counties is preferred.
Candidates must be able to use own vehicle for outreach and to transport eligible participants to non-emergency medical-related activities.
Candidates must be punctual, outgoing, communicate effectively (verbally and in writing), be committed to participant protection and privacy, and have high personal integrity.
No medical experience or training necessary; training will be provided.
Must pass project-specific competencies at end of training before beginning work.
