Job Description:

Responsible for conducting discipline specific and multidisciplinary evaluations to determine eligibility for the Northeastern Early Steps program as related to motor and other areas of development; Must prepare written recommendations and develop program plans to meet the individual needs of eligible children birth to 3 years of age; Must provide intervention/consultations as authorized on Individualized Family Support Plans and maintain files for each client; Must keep accurate records for billing and compliance.

Completes developmental evaluations, screenings, and assessments in collaboration with the multidisciplinary team for the purpose of determining eligibility, monitoring progress, updating IFSPs, and planning intervention.

Provides intervention/consultations as authorized on each child's Individualized Family Support Plan in the natural environment utilizing the Primary Service Provider approach

Develops Plan of Care and obtains authorization for services as required by third party payers and program policies.

Maintains and Submits intervention notes and billing information timely and accurately for records and payment.

Maintains clinical materials, supplies, and work area, And submits requests for replacement or repair as needed.

Advertised Salary:

$60,000 - $65,000 per year depending on education and experience.

Minimum Requirements:

Bachelor's degree from a recognized school of physical therapy, or completion of a Physical Therapy program. Licensed as a Physical Therapist in accordance with in accordance with Section 486, Florida Statutes and Rule Chapter 64B17, F.A.C.

Preferred Qualifications:

Degree as a physical therapist from an institution that has been approved for the training of physical therapists by the Commission on Accreditation for Physical Therapy Education (CAPTE).

One year of experience working with children birth to five with special needs.

Valid driver's license, Medicaid provider number, NPI number, Cleared Level 2 background screening, Healthcare Practitioner License.

This position requires that the Physical Therapist be able to operate effectively and with minimal supervision.
