The Department of Community Health and Family Medicine at the University of Florida’s College of Medicine is excited to announce an opening for a Community Health Worker (CHW) with the Mobile Outreach Clinic. This role supports homeless and unsheltered patients through hepatitis C (HCV) treatment by building trust, directly observing therapy, collecting treatment data and social determinants of health, completing patient assistance program applications, and placing and tracking referrals to local social service agencies. The CHW works closely with Peer Support Workers, clinical staff, program coordinators, and outreach teams to help patients complete treatment and achieve a cure. Mobile Outreach Clinic provides compassionate, community-based care to underserved individuals across Alachua County by meeting patients where they are and helping them access the services they need.
